Community Care Partners

VSHP has an innovative, proactive member-focused health assistance program to empower and transform the health of our members. We have established multiple intake points to coordinate integration of services, which includes physical health, behavioral health and environmental needs, to maintain a continuum of care. Our goal is to access a member’s situation, identify possible risks, and take prompt action to promote healthy outcomes.

VSHP has individuals geographically located to provide immediate support and response to the member, the Medical Home (PCP), and assist with care management functions. We are excited about this position for it’s ability to assess members quickly for identification of physical and behavior issues, and respond to the need for face to face empowerment as identified by our Care Management department. This identification allows VSHP to apply our resources in an appropriate and timely manner.
