VSHP Outreach Programs
VSHP strives to utilize our resources to the fullest to assist our members with achieving a better quality of life, and to assist in identifying health needs early, before serious health conditions develop or deteriorate. We have developed proven outreach strategies, which emphasize the importance of health care with a focus on the necessity of preventive care.
Community Outreach
VSHP focuses on community outreach and involvement to effectively target the the TennCare population. Our outreach staff is committed to creating an effective, multicultural environment for reducing barriers and developing healthy lifestyles for our members. The outreach staff is geographically located throughout the state of Tennessee and is knowledgeable of the community in which they serve. We are easily able to participate in events and work with members in their neighborhoods and homes.

Best Practice Network
VSHP wants to help children in state custody. VSHP has a special customer service team, called the Best Practice Network Unit (BPN).

CareSmart
CareSmart is the name of VSHP’s disease management program. CareSmart gives help to members who are chronically ill.

CaringStart
CaringStart works with members who are pregnant. Pregnant women are told about prenatal care that they need. All pregnant members receive a phone call. You can decide if you want to be part of the CaringStart program.

TENNderCare (EPSDT)
TENNderCare (EPSDT) works to improve health care for our members under age 21. It is vital that children get their check-ups on schedule. We make phone calls and send reminders to members who have not had their annual check-ups.

Volunteer Voices, our member advisory panel, is committed to improving the health of our members. Volunteer Voices allows us to host meetings and listen to our members and to use their ideas to improve our programs.

Community Care Partners
VSHP has an innovative, proactive member-focused health assistance program to empower and transform the health of our members. We have established multiple intake points to coordinate integration of services, which includes physical health, behavioral health and environmental needs, to maintain a continuum of care.
